Due to an overwhelming increase in the volume of forms that are needed for different things such as FMLA, disability, insurances, etc. there will now be a charge for any form that needs to be filled out if it is not provided to us at the time of your visit and/or is 3+ pages. 
If you bring your forms of 2 or less pages with you to your visit there will NOT be a charge.  However any forms that are 3 or more pages
